Jean Griswold Foundation National Caregiver Scholarship Program

The Jean Griswold Foundation National Caregiver Scholarship Program ("Scholarship Program") is a competitive tuition scholarship to be applied to tuition at any (US) nationally accredited institution. Scholarship awards will range between $2,000 to $10,000 as determined in the sole discretion of the Jean Griswold Foundation ("Foundation").

Eligibility: Individuals who serve as caregivers who are pursuing a first certification of a degree in the medical or non-medical care field of study; Legal age of majority in state of residence; US citizen or permanent resident or demonstrate lawful immigration status; Enrollment at any (US) nationally accredited institution for a degree or certificate program.

Nomination and Application Process: Individuals who are caregivers may submit an application for the Scholarship Program ("Applicant") with a supporting nomination from a supervising client, home care office or instructor ("Nominator"). Scholarship application with all supporting documentation, including the required nomination, must be submitted by the specified deadline. Incomplete applications will not be eligible for consideration. Selecting Scholarship Recipients: The Foundation will review all completed Scholarship applications beginning on or about April 27, 2023. The Foundation may schedule an interview prior to final selection and notification of Scholarship Program award recipients ("Scholarship Recipients") which an Applicant is required to participate and to which Nominators will be invited to participate.

Scholarship Program Terms and Conditions: The Applicant and the Nominator by submitting a scholarship application or nomination form, as may be applicable, unconditionally accepts and agrees (a) to comply with and abide by the Scholarship Program requirements as described herein ("Scholarship Terms and Conditions"); (b) to be bound by and acknowledge compliance with the Scholarship Terms and Conditions; and (c) to accept the decisions of the Foundation as final and binding in all respects.

Applicants selected to receive Scholarship Program awards ("Scholarship Recipients") agree that all Scholarship Program awards ("Scholarship Awards") must be applied only to tuition payments only within one (1) calendar year after notification of selection as a Scholarship Program recipient. Tuition does not include program or mandatory fees, surcharges, differential tuition, room and board or other expenses. Scholarship Awards may be used for program supplies or study abroad programs with written approval of the Foundation. The Foundation will disburse a Scholarship Award to the Scholarship Recipient's Bursar's student account within ten (10) business days of Scholarship Recipients student ID and institution mailing address. Students may transfer Scholarship Awards from one program to another upon written notice to the Foundation at info@jeangriswoldfoundation.org within fifteen (15) business days of program change. Scholarship Recipients may apply for Scholarship Awards for subsequent years. As a condition of submitting a scholarship application for the Scholarship Program to the Foundation, Applicants and Nominators agree to grant to the Foundation an unlimited, irrevocable worldwide, perpetual, non-excusive royalty free and fully paid up right and license, and permission to capture, stream, adapt, reproduce, display, create derivative works, and otherwise use and exploit by name, photograph, likeness, statements, biographical information, Scholarship Program application submission materials and other information, contributions, and materials which are related to or as a result of submitting a scholarship application to the Foundation for the Scholarship Program ("Materials"), in whole or part, in any all media, whether now or hereafter known of devised, for any and all purposes, including but not limited to promotional purposes in connection with the Scholarship Program or the Foundation's charitable activities. Such grant includes the subsequent uses of Materials that refer to or describe Applicant's or Nominator's participation or affiliation with the Scholarship Program or the Foundation, without review, credit, permission or further compensation or consideration. Applicant and Nominator, to the fullest extent permitted by applicable law, hereby expressly and irrevocably waives and releases all legal and equitable rights relating to all liabilities, claims, demands, actions, suits, damages, and expenses, including but not limited to claims for copyright or trademark infringement, infringement of moral rights, libel, defamation, invasion of any rights of privacy, violation of rights of publicity, physical or emotional injury or distress, or any similar claim or cause of action in tort, contract, or any other legal theory, now known or hereafter known or developed in any jurisdiction throughout the world (collectively, the "Claims") arising directly or indirectly from (a) Applicant's or Nominator's participation in the Scholarship Program; (b) the Foundation's exercise of their rights under these terms and conditions; or (c) the production, exhibition, advertising, promotion, exploitation, or other use of the Materials, and whether resulting in whole or in part by the negligence of the Foundation or any other person. Applicants and Nominators covenant not to make or bring any such Claim against the Foundation and their agents, employees, and representatives, successors and assigns (collectively, the "Releasees") and forever release and discharge the Releasees from liability under such Claims. If requested, Applicants and Nominators will sign any additional documentation by any Releasees regarding or confirming any releases of liability hereunder.
